In 2013, the Lozano and Elizondo families first started San Jose Health Care with a passion for providing excellent in-home services to the seniors in our community who have taught and continue to lead the members of our society. Our team wanted to continue providing excellent care to families and their loved ones every step of the way, and so San Jose Hospice Care began.
Our loved ones mean the world to us, so we strive to produce top-notch hospice services to our patients as if they were our own family. Our team is committed to giving our patients comfort and support for their remaining time with us. At the same time, we bring comfort and care to the patient’s families.

Our staff is the heartbeat of San Jose Hospice; without them, we would simply not function. To have such a fantastic team, we follow protocol to ensure that we only hire the best. These protocols include:

Our team members constantly train to deliver the best, most up-to-date care possible. These training include:

Our patients and their families have complete control over choosing which caretaker they would feel most comfortable in their care. Patients tend to choose those who they can be friends with, and families have the opportunity to familiarize themselves with their new friends.

Our Philosophy

We acknowledge dying is natural and inevitable when curative therapy is no longer succeeding. We are not looking to hasten or prolong the dying process. We believe that providing quality hospice care and catering to individual needs will allow dying patients to live out their best days until their last. Losing a loved one is never easy. Our team is here for your family during their illness and after their passing.
